From the ones I have seen, they are cosmetic only. Not a substantial source of heat.

I've put an ethanol burner in my old non-functioning gas fireplace as the replacement or fixing cost for the old unit costs much more than I anticipated.  Overall, I like the ethanol burner very much as it put out surprising heat and it presents a very relaxing atmosphere.  Having said that, I wouldn't go as far as saying it heats as efficiently as central heating would.  There are some people concerning about the odor.  I'd say it's up the fuel brand you'd use.  I currently use e-NRG and it's fine.  The fuel is alcohol so you'd smell it when you open the container.  During burning it's barely noticeable.  The only time you'd smell something is when the flame goes out, and this depends on what brand you use.  So far, I'm happy with it.  There isn't much cleaning to do.  I do leave a crack on the window as the flame does eats up oxygen in the room.  I got the burner at http://www.andra5.com/fireplace/bio-ethanol-burner/  if you're interested.  Hopefully this helps.  Here is a picture of it during one of my first uses.
